Provide residents with information and a referral list for community agencies, services and programs.

Work with agencies and volunteers to help with personal finance or insurance issues

Assist residents with understanding bills, medical forms, and filling out eligibility, entitlement program and legal forms.

Help residents arrange for housekeeping, shopping, transportation, Meals On Wheels, cooking and laundry assistance, and legal advocacy.

Arrange for socialization activities, library deliveries, and computer training, along with helping build informal support networks with other residents.

Available to residents to answer questions or address concerns.

Works cooperatively with the Nurse.

Advocate to ensure health related issues are taken care of.

Coordinate regular educational sessions.
